Surgical Pathway to Weight Loss Surgery

The surgeons at Central Carolina Surgery recognize that choosing the right procedure will have a significant impact on your life. We would like to help you make the right decision for you, your family, and your lifestyle. The following steps will help guide you through this process.

If you are searching for a weight loss surgeon in the Triad, your search is over.

1. Attend required free seminar either in person or online to start your weight loss journey.

2. Complete and turn in “Patient Profile Form” and a copy of the front/back of your insurance card.

At the seminar, you will be given a packet that contains forms that you will need to complete as well as additional information about surgery. You may call your insurance carrier and verify if you have bariatric surgery coverage.

3. Initial Consultation with the surgeon

After the Patient Profile Form and the insurance card(s) are received, the Bariatric Coordinator will verify your coverage/benefits for bariatric surgery and contact you to schedule your appointment with your surgeon. Please bring the following completed items to your consultation appointment with your surgeon:

  • Patient Medical History Form
  • Patient Self-Report Weight Loss History Form
  • List of current medications
  • Insurance card(s)
  • Letter written by you explaining why you want to have weight loss surgery
  • Photo ID

4. Consultations and Testing

If your surgeon determines you are a candidate for surgery, the Bariatric Coordinator will meet with you (usually same day) to schedule appointments for you including:

  • Nutritional consultation (There is a non-refundable $100 processing fee charged by the Nutrition Diabetes Management Center)
  • Psychological consultation
  • Testing necessary to evaluate you for surgery such as labs, upper GI, x-rays, and sometimes medical clearance (testing is ordered based on individual need)

The Bariatric Coordinator will communicate all appointment dates/times to you usually at the time of your appointment with the surgeon. Failure to keep appointments will result in surgery delays.

5. Insurance Approval

Once you have completed all your appointments and tests, your documentation will be forwarded to the Bariatric Coordinator to begin the insurance prior approval process. This process can take up to four weeks. Please note: The Letter of Medical Necessity form from your primary care doctor must be received by the Bariatric Coordinator prior to your request being sent to insurance. This is an insurance requirement.

6. Preoperative Appointment

Your preoperative appointment with your surgeon will be made after prior approval has been received from the insurance carrier and you have paid your deposit to Central Carolina Surgery. Normally the appointment will be made for 7 to 14 days before surgery.

Your deposit (including any co-pays, deductibles, etc) must be paid in full prior to scheduling. Your surgery will ONLY be scheduled if all financial obligations have been made. Please remember to keep a copy of all documents for your records.
